目的 探讨弥漫性胶质瘤瘤周水肿组织脑和肌肉ARNT样蛋白1(BMAL1)的表达及其临床意义。方法 选取2021年6月至2023年1月手术切除的64例弥漫性胶质瘤瘤周水肿组织,免疫组化法检测瘤周水肿组织BMAL1表达;术前进行脑MRI检查,计算水肿指数评估瘤周水肿程度。结果 64例均伴有瘤周水肿,其中轻度水肿20例,中度水肿9例,重度水肿35例;瘤周水肿组织BMAL1高表达39例,低表达25例。瘤周水肿组织BMAL1免疫组化染色评分与水肿指数呈明显正相关(r=0.408;95% CI 0.179~0.594;P<0.001)。多因素logistic回归分析显示,BMAL1高表达是弥漫性胶质瘤并发重度瘤周水肿的独立影响因素(OR=5.558;95% CI 1.897~8.467;P<0.001)。结论 弥漫性胶质瘤瘤周水肿组织BMAL1表达水平与瘤周水肿程度呈正相关,提示BMAL1可能参与弥漫性胶质瘤瘤周水肿的发生、进展。
Abstract:
Objective To investigate the expression of brain and muscle ARNT-like 1 (BMAL1) in the peritumoral edema tissues of diffuse gliomas and its clinical significance. Methods The BMAL1 expression levels were detected by immunohistochemical staining in the peritumoral edema tissues obtained from 64 patients with diffuse gliomas who underwent surgical resections between June 2021 and January 2023. Preoperative brain MRI was conducted, and the edema index was calculated to assess the severity of peritumoral edema. Results All 64 cases presented peritumoral edema, with 20 mild, 9 moderate, and 35 severe edema. High BMAL1 expression was observed in 39 patients, and low expression in 25. The immunohistochemical staining score for BMAL1 in the peritumoral edema tissues showed a significant positive correlation with the edema index (r=0.408; 95% CI 0.179~0.594; P<0.001). Multivariate logistic regression analysis revealed that high BMAL1 expression independently predicted severe peritumoral edema (OR=5.558; 95% CI 1.897~8.467; P<0.001). Conclusions The level of BMAL1 expression in peritumoral edema tissues is positively correlated with the degree of peritumoral edema in diffuse gliomas, suggesting potential involvement of BMAL1 in the occurrence and progression of peritumoral edema.
