摘要:
目的 探讨椎管内侵袭型血管脂肪瘤的诊断和手术治疗。方法 回顾性分析11例椎管内侵袭型血管脂肪瘤患者的临床资料，均行显微手术治疗。结果 肿瘤均获得全切。术后未出现并发症。术后病理组织学检查均符合血管脂肪瘤。随访9~38个月，所有病例神经功能均明显改善；末次随访时Frankel分级均恢复至E级，无脊柱畸形及不稳，复查MRI无肿瘤复发。结论 椎管内侵袭型血管脂肪瘤手术切除效果佳，肿瘤侵袭性生长方式决定手术策略。
Abstract:
Objective To summarize the experiences in diagnosing and surgically treating the infiltrating spinal angiolipomas. Method The clinical data of 11 patients with infiltrating spinal angiolipomas were analyzed retrospectively, including the general data, clinical manifestation, imaging data, and surgical method and effects. Results Total resection of the tumors was achieved in all the patients. No complications were observed during and after the operation in all the patients. All the lesions were diagnosed as angiolipomas by histopathological examination. The following-up from 9 to 38 months showed that the neurological function was significantly improved in all the patients, in whom, the neurological function recovered to Frankel grade E and there were not the deformity and unstability of spinal column and recurrence of tumors. Conclusions The curative effect of surgery on infiltrating spinal angiolipomas is good. The strategy of surgery for infiltrating spinal angiolipomas depends on the tumorous growth pattern.
