摘要:
目的 探讨静息态功能MRI(rs-fMRI)在脑震荡综合征诊断中的价值。方法 选择2013年2月到2016年5月在我院诊治的脑震荡综合征36例作为观察组,选择同期在我院进行体检的健康人36例作为对照组。两组都进行脑震荡综合征问卷调查表(RPSQ)评分,同时进行rs-fMRI检查。结果 观察组RPSQ评分[(11.44±2.19)分]明显低于对照组[(17.22±1.93)分;P<0.05]。与对照组相比,观察组双侧扣带回前部皮层在Slow-5亚频段分数低频振幅(fALFF)显著增高(P<0.05),而显著减低的脑区为双侧枕中回和楔前叶(P<0.05);观察组左侧颞下回在Slow-4亚频段fALFF显著增高(P<0.05)。观察组扣带回前部,左侧颞下回fALFF与RPSQ评分水平呈显著负相关(r分别为-0.456,-0.547;P<0.05);而双侧枕中回和楔前叶fALFF与RPSQ评分水平呈明显正相关(r分别为0.598,0.585;P<0.05)。结论 rs-fMRI检查能反映脑震荡综合征脑白质的轻微异常情况,可提供病理层面的检测客观证据。
Abstract:
Objective To investigate the value of resting state functional MRI (rs-fMRI) to the diagnosis of postconcussion syndrome. Methods Thirty-six patients with postconcussion syndrome treated in our hospital from February, 2013 to May, 2016 served as the observation group, and 36 healthy subjects receiving health examination in our hospital at the same period served as the control group. The postconcussion syndrome questionnaire (PCSQ) investigation was performed in all the subjects. The relationship of PCSQ scores with rs-fMRI was analyzed. Results The PCSQ scores [(11.44±2.19) points] were significantly lower in the observed group than those [(17.22±1.93) points] in the control group (P<0.05). The Slow-5 subfrequency fractional amplitude of low frequency fluctuation (fALFF) of bilateral anteriomedial cingulate cortexes was significantly higher (P<0.05), and the fALFF of the bilateral medial occipital gyri and precuneuses were significantly lower in the observation group than those in the control group (P<0.05). The Slow-4 subfrequency fALFF of the left inferior temporal gyrus was significantly higher in the observation group than that in the control group (P<0.05). In the observation group, Spearman analysis showed that the fALFF of the anterior cingulate gyrus and left inferior temporal gyrus were negatively related to the PCSQ scores (r=-0.456, r=-0.547, respectively; P<0.05), and the fALFF of bilateral anteriomedial occipital gyri and precuneuses were positively related to the PCSQ scores in the observed group (r=0.598, r=0.585, respectively; P<0.05). Conclusion The rs-fMRI can reflect the mild abnormality of the cerebral white matter in the patients with postconcussion syndrome and therefore it can provide the objective evidence of the pathological level, and it is of value to the diagnosis in the patients with postconcussion syndrome.
