摘要:
目的 探讨脑室外引流术(EVD)在颅内破裂动脉瘤急诊开颅夹闭术中的应用效果。方法 回顾性分析2013年10月至2017年6月急诊开颅夹闭术治疗的62例颅内破裂动脉瘤(Hunt-Hess分级Ⅲ~Ⅴ级)的临床资料。术前行EVD治疗35例(EVD组),未行EVD治疗27例(非EVD组)。术后随访6个月至2年。结果 EVD组症状性脑血管痉挛发生率(31.4%,11/35)显著低于非EVD组(59.3%,16/27;P<0.05)。EVD组预后良好率(71.4%,25/35)明显高于非EVD组(44.4%,12/27;P<0.05)。结论 对于颅内破裂动脉瘤,夹闭术前进行EVD,可以有效降低颅内压力,缓解因脑水肿引起的颅内压增高,减少术后并发症,改善病人预后。
