摘要:
目的 观察伽玛刀治疗硬脑膜动静脉瘘(DAVF)的疗效。方法 回顾性分析2008年10月至2016年4月采用伽玛刀治疗26例DAVF的临床资料,其中单纯伽玛刀治疗14例,伽玛刀治疗前经历一次或多次血管内治疗12例。用50%~60%等剂量曲线包绕病灶边缘,周边剂量15~22 Gy,平均18.9 Gy;中心剂量30~44 Gy,平均37.5 Gy。26例治疗后随访31~149个月,平均81.1个月。结果 14例DSA复查证实瘘口完全闭塞;7例症状消失,MRI/MRA或CT/CTA证实引流静脉变细或消失,但拒绝DSA检查;DSA证实未闭塞5例中,2例症状好转,继续观察,2例行血管内治疗,1例行开颅手术治疗。26例治疗后病情稳定,无新增症状。结论 伽玛刀治疗DAVF是一种安全、有效的方法。对侵袭性DAVF,血管内或手术治疗后再行伽玛刀治疗是一种可供临床选择的比较理想的方案。
Abstract:
Objective To observe the curative effect of gamma knife on dural arteriovenous fistulas (DAVFs). Methods Of 26 patients with DAVFs, 14 were treated only by gamma knife radiosurgery and 12 patients were treated by gamma knife radiosurgery after the endovascular embolization. According to the Cognard classification, DAVFs belonged in type Ⅰ in 7 patients, type Ⅱ in 12 and type Ⅲ in 7. The targets were covered by 50% to 60% isodose levels. And the average prescribed margin target doses were 18.9 Gy(ranging from 15 to 22 Gy). All the patients were followed up from 31 to 149 months(mean, 81.1mouths). Results MRI and CT showed that the drainage-vein disappeared in 7 patients and DSA showed that the DAVFs were obliterated in 14 patients and the DAVFs were not occluded in 5 patients during the following up. No complications occurred in all the patients. Conclusion Gamma knife is a safe and effective method to treat the DAVFs, especially aggressive DAVFs.
