摘要:
目的 探讨3D外视镜辅助前路颈椎间盘切除融合术(ACDF)治疗脊髓型颈椎病的效果。方法 回顾性分析2019年1~6月3D外视镜辅助ACDF治疗的15例脊髓型颈椎病的临床资料。术后随访12~16个月,平均(14.40±1.45)个月,采用日本骨科协会(JOA)评分评估神经功能。结果 所有病人均在3D外视镜辅助下顺利完成手术,手术时间90~160 min,平均(111.33±17.88)min;术中出血量10~100 ml,平均(44.67±28.50)ml。末次随访JOA评分[(14.27±1.03)分]较术前[(9.60±1.68)分]明显增加(P<0.05)。神经功能改善率在50%~80%,平均(63.41±9.54)%;其中优3例,良12例,优良率为100%。所有病人均未出现手术相关并发症。结论 3D外视镜辅助ACDF治疗脊髓型颈椎病,能够提供良好的放大和照明效果,是一种安全、有效的治疗方法。
Abstract:
Objective To explore the clinical efficacy of 3D exoscope-assisted anterior cervical discectomy and fusion (ACDF) for the the patients with cervical spondylotic myelopathy (CSM). Methods The clinical data of 15 patients with CSM who underwent 3D exoscope-assisted ACDF from January 2019 to June 2019 were analyzed retrospectively. The postoperative follow-up ranged from 12 months to 16 months, with an average of (14.40±1.45) months. The Japanese Orthopaedic Association (JOA) score was used to evaluate clinical outcomes. Results The operations were successfully completed in all the patients with the assistance of 3D exoscope. The mean operation time was (111.33±17.88) minutes and the average blood loss was (44.67±28.50) ml. The JOA score was significantly higher at the last follow-up [(14.27±1.03) points] than that [(9.60±1.68) points] before the operation (P<0.05). The improvement rate of neurological function ranged from 50% to 80%, with an average of (63.41±9.54)%. Eexcellent improvement of neurological function was achieved in 3 patients and good in 12, and the excellent and good rate was 100%. There was no postoperative complications related to the surgery. Conclusions The 3D exoscope can provide excellent magnification and illumination. 3D exoscope-assisted ACDF is a safe and effective procedure in the treatment of patients with CSM.
