摘要:
目的探讨CT高密度垂体Rathke囊肿的MRI影像学特征。方法回顾性分析2013年1月至2018年1月手术治疗的1 104例垂体病变的临床资料,其中Rathke囊肿186例,而术后病理证实为垂体Rathke囊肿、术前CT为高密度的Rathke囊肿9例。结果9例CT呈高密度的垂体Rathke囊肿中,垂体柄上型5例,垂体柄下型4例;MRI T1像呈等信号6例、高信号3例,T2像呈等信号5例、低信号2例、高信号2例;垂体柄上型囊肿囊壁未见强化,垂体柄下型囊肿周围呈现明显的环形强化征。结论CT呈高密度的垂体Rathke囊肿可分为垂体柄上型和垂体柄下型,以前者多见;该类囊肿MRI平扫缺乏恒定的影像学信号,动态增强影像显示垂体柄上型囊壁无明显增强,垂体下型囊壁呈现明显的环形强化征。
Abstract:
Objective To explore the MRI features of the pituitary Rathke cleft cysts (RCC) with the appearance of high density on CT image. Methods A retrospective analysis was performed on the clinical data of 1 104 patients with pituitary lesions who underwent surgery from January 2013 to January 2018. Results Postoperative pathological examination showed RCC in 186 patients of 1 104 patients with pituitary lesions. Of 186 patients with RCC, 9 patients showed the appearance of high density on preoperative CT images. The MRI showed supra-pituitary stalk RCC in 5 patients and sub-pituitary stalk RCC in 4; isosignal intensity on T1-weighted images in 6 patients and high signal intensity in 3; isosignal intensity on T2-weighted images in 5 patients, low signal intensity in 2 and high signal intensity in 2; no enhancement in supra-pituitary stalk RCC and obvious ring-shaped enhancement in sub-pituitary stalk RCC. Conclusions There is no specified signal of MRI plain scan for the RCC with the appearance of high density on preoperative CT image. On dynamic enhanced images, the supra-pituitary stalk RCC show no enhancement and sub-pituitary stalk RCC show obvious ringshaped enhancement.
