目的 探讨笑气滥用导致的周围神经病(PN)的肌电图特征。方法 回顾性分析2019年5月至2023年5月收治的124例笑气滥用的临床资料。采用多伦多临床评分系统(TCSS)评估PN,TCSS评分≥6分判定为PN。结果 124例中,78例发生PN,发生率为62.90%(78/124)。与无PN病人相比,PN病人腓总神经、正中神经和尺神经MCV、CMAP波幅明显降低(P<0.05),腓浅神经、正中神经和尺神经SCV、SNAP波幅明显降低(P<0.05)。多因素logistic回归分析显示,腓总神经、正中神经和尺神经MCV、CMAP波幅降低,以及腓浅神经、正中神经和尺神经SCV、SNAP波幅降低是笑气滥用病人发生PN的危险因素(P<0.05)。根据这些危险因素构建预测模型具有良好的预测价值,ROC曲线下面积为0.845(95% CI 0.790~0.900;P<0.001)。结论 笑气滥用导致的PN肌电图会发生异常改变。笑气滥用病人进行肌电图检测有助于早期识别PN。
Abstract:
Objective To explore the electromyographic characteristics of peripheral neuropathy (PN) caused by nitrous oxide abuse. Methods A total of 124 patients with nitrous oxide abuse treated in our hospital from May 2019 to May 2023 were selected as subjects. PN was diagnosed using the Toronto Clinical Scoring system (TCSS), and PN was defined as TCSS score ≥6 points. Results PN occurred in 78 of the 124 patients (62.90%, 78/124). Compared with patients without PN, MCV and CMAP amplitude of common peroneal nerve, median nerve and ulnar nerve were significantly decreased in PN patients (P<0.05), SCV and SNAP amplitude of superficial peroneal nerve, median nerve and ulnar nerve were significantly decreased (P<0.05). Multivariate logistic regression analysis showed that decreased MCV and CMAP amplitude of common peroneal nerve, median nerve and ulnar nerve, and decreased SCV and SNAP amplitude of superficial peroneal nerve, median nerve and ulnar nerve were risk factors for PN in nitrous gas abuse patients (P<0.05). The predictive model constructed according to these risk factors had good predictive value, and the area under ROC curve was 0.845 (95% CI 0.790~0.900; P<0.001). Conclusions There are abnormal changes in electromyogram of PN caused by nitrous oxide abuse. Electromyogram detecting is helpful for early identification of PN in patients with nitrous oxide abuse.
