摘要:
目的 探讨胶质瘤病人血清外泌体miRNA-30a-5p的表达水平,及其在胶质瘤诊断及预后评估中的价值。方法 选取2014年1月至2015年12月收治的胶质瘤60例为研究对象,另选取健康体检者40例为对照。RT-PCR检测血清外泌体miRNA-30a-5p表达水平。用受试者工作特性(ROC)曲线分析miRNA-30a-5p诊断胶质瘤的价值。用Kaplan-Meier法绘制生存曲线,采用log-rank检验;多因素Cox比例风险回归模型分析预后危险因素。结果 胶质瘤病人血清外泌体miRNA-30a-5p表达水平明显高于健康人(P<0.001)。ROC曲线分析显示,血清外泌体miRNA-30a-5p诊断胶质瘤的曲线下面积为0.901(95% CI 0.812~0.986),最佳截断值为0.601,此时miRNA-30a-5p诊断胶质瘤的灵敏度和特异度分别为78.05%和93.55%。术后60例均得到有效随访,随访时间平均(28.39±3.25)个月。Kaplan-Meier分析结果显示,血清外泌体miRNA-30a-5p低表达病人总体生存时间高于高表达病人(P<0.001)。多因素Cox比例回归风险模型分析结果显示血清外泌体miRNA-30a-5p高表达是胶质瘤病人不良预后的独立影响因素(P<0.05)。结论 血清外泌体miRNA-30a-5p对胶质瘤的诊断和预后评价均有一定价值
Abstract:
Objective To investigate the expression level of serum exosome miRNA-30a-5p in glioma patients and its value to the diagnosis and prognostic evaluation of glioma. Methods The expression of miRNA-30a-5p in exosomes was detected by RT-PCR in 60 glioma patients and 40 healthy people. Receiver operating characteristic (ROC) curve was used to analyze the value of miRNA-30a-5p to the diagnosis of gliomas. Kaplan-Meier method and log-rank test was used to analyze the overall survival time of the patients with glioma. Cox proportional risk model was used to analyze the prognostic factors of the patients with glioma. Results The expression level of serum exosome miRNA-30a-5p in glioma patients was significantly higher than that in healthy people (P<0.001). ROC curve analysis showed that the area under curve was 0.901 (95% CI 0.812~0.986), and the best cut-off value was 0.601. The sensitivity and specificity of serum exosome miRNA-30a-5p to the diagnosis of glioma was 78.05% and 93.55%, respectively. All these 60 glioma patients were followed up effectively after the operation, with an average follow-up time of (28.39±3.25) months. Kaplan-Meier analysis showed that the overall survival time of patients with low expression of serum exosome miRNA-30a-5p was significantly higher than that of patients with high expression of serum exosome miRNA-30a-5p (P<0.001). The results of multivariate Cox proportional regression risk model analysis showed that the high expression of serum exosome miRNA-30a-5p was an independent risk factor for the poor prognosis of glioma patients (P<0.05). Conclusion Serum exosome miRNA-30a-5p has certain value to the diagnosis and prognosis evaluation of human glioma
